Duties of Resident Health Manager (Nurse) at CovaiCare Retirement Communities

 

On the occupation of the dwelling by any resident, ensure that the Doctor of the retirement community or the Doctor from Covai’s PolyCare visits the residents and take the following actions for every individual:

 

  1. Record the usual parameters of the individual (BHA) both on the system and the medical file.
  2. Check his/her prescription and if it is multiple prescriptions, make one single prescription.
  3. Find out the serious illnesses they have like Diabetes, Hypertension, Kidney or Heart conditions, etc., and who their primary doctor is. This must be recorded
  4. Open a Medical file for the resident and file all his medical record, including the past records in chronological order –latest reports on top
  5. If the resident moves in from another place to CovaiCare’s retirement community, depending upon the illness, identify a specialist Doctor in that location and make sure he/she is examined, and record updated. This would be useful in emergencies
  6. Enter his or her data in the computer system at the Medical Centre after assigning a number for each patient.
  7. A Medical file numberfor each resident will be given by VP (Care). If a resident leaves the community permanently, his or her record will be kept in the archive at Covai’s PolyCare and erased at the respective community. His or her medical file will be handed over to them or their next of kin as applicable. New Number will be assigned to the new resident by VP (Care).
  8. The Medical File No of the resident will be prominently written on the cover page of the Medical File along with the following details:
    1. Name
    2. Date of Birth Blood Group
    3. Address – (Villa/Apartment No with Block, Project Name)
    4. Mobile No Landline No Email ID
    5. Emergency Contact: Name,Mobile No
    6. Spouse Name (If applicable)
    7. Specialist Doctor(s ) – Name, Mobile no, Hospital
    8. Allergies:
    9. Special Instructions                                                                                  

     

  9. Whenever there is any medical attention given to any resident, ensure that data is passed onto Covai’s PolyCare and the same is also updated in the system at the Medical Centre and Covai’s PolyCare. This should be in a format to be issued by the VP (Care). Get confirmation in writing from the CRM of Covai’s PolyCare by email that the data has been updated.
  10. For each resident, note the medicines prescribed, stock available, organise medicine box for each dose and help the senior in giving the medication as routine if asked for. Also, ensure that any change in medicines is implemented and old medicine which could have been discontinued by the Doctor is removed from the Medicine Box of the resident immediately.
  11. Note down in the system both at Covai’s PolyCare and at the Medical Centre, the next date of refill of medicines. Two days before the date of refill, Covai’s PolyCare will send notice to the resident and the RHM. RHM will also speak to the client and arrange the refill of medicine from the chemist. The confirmation message of this medicine refill would be submitted by the RHM to the CRM of Covai’s PolyCare
  12. If a patient is out of the station, call him or her or if they are overseas, send an email and remind him/her and see to it that he/she buys the medicines and fills up the medicine box on time. In fact, if the concerned resident, be cautioned on his or her refill of medicines before their departure, it would be welcome.
  13. Before ordering medicines check with the resident the quantity and type of medicine to be ordered. This is to find out if the resident has skipped any medicine and if so, the RHM has to remind such residents to take medicine on a daily basis. This fact of skipping or missing medicine should be recorded and RHM should take the extra initiative to ensure that this is not repeated.
  14. For those seniors who because of age are forgetful, RHM will ensure that she personally administers the medicine to them. Depending upon the state of forgetfulness or Dementia or Depression, medicines may be kept only in the Medical Centre and administered by RHM on daily or dosage basis.
  15. In case of Super Seniors or those with any disability or suffering from memory loss or Depression or Parkinson or are in bed with disability (short or long term), the RHM will personally monitor their medicine intake and other therapies like Physio etc., The RHM will record the same in the medical file, update the system in the Medical Centre and also pass on the information to Covai’s PolyCare and get the confirmation.
  16. When a resident is moved to Covai’s Assisted or Memory Care Centre, the Medical file will be handed over to the Nurse at Care Centre and a signature obtained. On return from the Care Centre similar exercise will be followed
  17. The Senior Health Manager will audit the function of RHM and take corrective actions if there is any slip-up.
  18. If any patient reports sick or is evacuated to hospital, the RHM will inform the Senior Health Manager (SHM) who in turn would keep the Senior Medical Officer (SMO) and local Medical Officer (MO) informed. Daily Health Status Report of such residents admitted in hospital or detained in our Medical Centre for observation would be sent to SHM, SMO, MO, VP (Care), VP(Services) and MD.
  19. The RM or RHM must visit the patient(s) in any hospital every day until discharge and file a report to those mentioned in (p) above
  20. If any resident is single and wants caregiver’s support at the hospital during and after admission, RM will ensure that such assistance is provided and charged. For this VP (Care) and RMs will have to tie up with a good agency like Portea and have an MOU signed and rates negotiated in advance. The details will also be informed to all residents officially
  21. The date of any sick report and reason thereof or hospital visit/admission and the reason thereof would be recorded in the Medical File, the system at the Medical Centre of the retirement community and at Covai’s PolyCare
  22. RHM would also facilitate an examination by the Consulting Specialist Doctor either on periodic check-up or as requested by the resident. Arrangements for going to the hospital will be coordinated with the RM
  23. The RHM would make sure that the Medical File is taken by the patient or his attendant to the hospital if admitted.
  24. The RHM would also inform the consulting Specialist doctor about the patient’s evacuation to the hospital in an emergency. The RM would inform the patient’s Next of kin and be in touch with them as necessary.
  25. The RHM in consultation with VP (Care) and SMO would arrange for the Flu injections. SMO will be responsible for the same for all retirement community.

The RHM would also be responsible for Telemedicine (2CT), visit of authorized Physiotherapist to those who are in need of the same.
